Social Services Meet the People

Our Social Services Team focuses on understanding the unique needs and challenges of our patients and residents. They provide emotional support to patients and their families, facilitate discharge planning to ensure smooth transitions between hospital care and swing bed programs.  They collaborate closely with healthcare teams to coordinate ongoing care and community resources. Their role is pivotal in advocating for our patients’ well-being and enhancing their quality of life during their time with us at OMHC.
